Job Description

As a Bereavement Coordinator you need to know how to:

  • Establish and provide ongoing coordination of Hospice Bereavement Services according to the Hospice Conditions of Participation and the Hospice Standards for Accreditation for the Joint Commission on Accreditation for Health Care Organizations (JCAHO)
  • Meet the bereavement needs of the patients/families/significant others and support and resources for staff
  • Provide and coordinate bereavement services to Hospice patients and staff by investigating and maximizing available resources to assist in this endeavor. These resources being: interns, students, PRN staff, other discipline professionals (social workers), and volunteers
  • Demonstrate the ability to work in partnership with the Hospice staff to contribute to the multidisciplinary approach to patient care planning and delivery with focused attention to the timely intervention in the plan of care with early bereavement support, anticipatory grief education and support and post death bereavement care
  • Take responsibility for the overall compliance of the various aspects of the Hospice bereavement Services (i.e.) bereavement assessment completion and follow-up in collaboration with social worker staff, information to volunteers for timely telephone support, scheduled mailings done by volunteers and administrative staff of Hospice, 1:1 counseling, support group facilitators scheduling and activity, etc
  • Provide professional bereavement support and services through consistent and effective assessment, planning, intervention and evaluation.

Minimum Qualifications

  • Masters degree in social work or counseling
  • Licensure (LPC or LCSW)
  • Two years of related experience within a healthcare organization
  • Knowledge based understanding of the process of death and dying and grief and loss
  • Demonstrated experience and knowledge of group process and facilitation.

Preferred

  • Two or more years as a Hospice Bereavement Coordinator
  • Experience in curriculum development
  • Documented experience in supervising support staff especially volunteers

Physical Requirements:

Interact with others by effectively communicating, both orally and in writing.- and -Operate computers and other office equipment requiring the ability to move fingers and hands.- and -See and read computer monitors and documents.- and -Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.- and -May require lifting and transporting objects and office supplies, bending, kneeling and reaching.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$24.29 - $38.26
