Job Summary
Under general supervision, the Denver Health Pediatrics at Denver Public Schools School-Based Health Center (SBHC) Social Work Program Coordinator (SWPC) is responsible for facilitating and coordinating all aspects of program implementation for the in-person PATCH Teen Educator team and Healing Centered Engagement resiliency program. Creates authentic relationships with all youth; plan and execute ongoing in-person programming for a group of 10-12 high school aged students; schedules, organizes, and leads all workshops for teen and health care provider audiences; and encourages continuous quality improvement for expanded reach and program growth throughout Denver Health and the Denver Community. The role includes delivering care to an assigned group of patients within SBHC's to promote resiliency work via a Healing Centered Engagement approach. Coordination involves assessment of care needs, planning, support, and evaluation of member care and related outcomes. Activities to be performed are screening and assessment of medical, behavioral health and social determinants needs and gaps in care, collaboration with the Member to develop a care plan with SMART goals, scheduled outreach to support member in achieving their goals and supporting the Members patients' self-efficacy to navigate systems. SWPC communicates closely with the Member's care team inclusive of: the Member, designated health representatives, primary care provider, behavioral health providers and other care coordinators involved with the Member's Patient's care. Encounters occur face to face, telephonically, and electronically with members patients in clinic and in other community-based settings. SBHCs. SWPC will also contribute to program sustainability through assisting with identifying potential funding oportunities and grant writing.

Position is an active contributor in the development of systems (e.g.e.g., work flowsworkflows, clinical pathways, assessments) to improve the care of assigned member patient populations. Helps ensure compliance with state, federal and third party payor requirements.

Essential Functions:

  • Manage and coordinate the design and implementation of Teen Educator and Healing Centered Engagement Programs. (20%)
  • Possess an advanced understanding of health equity and social justice and be committed to integration into all levels of the program. (10%)
  • Ensures that all efforts are equitable, inclusive, and support diversity. (5%)
  • Utilize Lean principles, project management and quality improvement to improve youth health and grant program outcomes. (10%)
  • Create a culture of comfort, trust, openness, and support among Teen Educators, supervisees and patients. (10%)
  • Plan and facilitate annual 20-hour Teen Educator Training (5%)
  • Plan and facilitate bimonthly evening Enrichment Meetings for Teen Educators to further educate, engage, and inspire youth (5%)
  • Lead workshop promotion and coordination including marketing, outreach, recruitment, and scheduling (5%)
  • Build and maintain community relationships including DPS and outside agencies. (10%)
  • Develop partnerships to leverage program and grant activities and funding. (10%)
  • Develop and manage work plans, activities and implementation. (5%)
  • Able to adapt to changes in work environment and manage competing demands. (5%)


Education:

  • Bachelor's Degree Required
